1.COVID-19 vaccination starting from spring 2023 (for 3rd, 4th, 5th and 6th doses)

Subject Person

Persons who have received the primary series (1st and 2nd doses) of COVID-19 vaccine and fall under any of the following:
①Persons aged 65 or older
②Persons aged 5 - 64 with underlying diseases
③Healthcare workers, workers in care homes etc.
*If you are applicable to ② or ③ and want to receive the vaccine, please apply for the issuance of vaccination ticket.

Vaccination Period

Mon, May 8th - Thu, Aug. 31st

Vaccination Interval

3 or more months from the previous COVID-19 vaccination

2. Type of Vaccine

Pfizer or Moderna bivalent COVID-19 vaccine targeting Omicron variants

Persons who cannot get vaccinated

      • Persons who have a fever
      • Persons who have a serious acute illness
      • Persons who have experienced life-threatening allergic reaction

Persons who need caution

      • Persons who are under treatment of heart failure, liver failure, kidney failure, blood failure, immune deficiency and so on
      • Persons who have experienced allergic reaction against medicines and foods
      • Persons who has blood failure  etc.

3. About COVID-19 Vaccination Ticket

We send the vaccination ticket which is necessary to get the COVID-19 vaccine by post. You need the ticket to get vaccinated. Please keep the ticket carefully until your vaccination day.

4. How to Make a Vaccination Reservation

Reservation is necessary for the vaccination.
You can make a vaccination reservation by phone or online.
*If you find it difficult to make a reservation online, please contact  the call center.
*If you have difficulty communicating in Japanese on the day of your vaccination, please contact the call center in advance.

5. Mass Vaccination Venue (12 years old or older)

*The vaccination at Minami Community Center is until Sat, Jun. 24th.
*Please make a reservation after receiving the vaccination ticket.
*For vaccination shcedule, please contact the call center at 092-580-1892.
*Notes
・Due to the delay of the COVID-19 vaccine supply, the vaccination might be cancelled. We will inform you of the cancellation 4 days before the day that vaccination will be conducted on the HP of Onojo City and the city vaccination reservation website. We will send a post card to persons who cannot take a vaccine due to the cancellation.
・When we cancel the vaccination due to a natural disaster, we will inform you of the information on the HP of Onojo City and the city vaccination reservation website as needed.

Vaccination Procedure

Before you go to your vaccination venue

      • Please read ''Notification about COVID-19 Vaccination'' and  ''Instructions for COVID-19’’ which are enclosed with the COVID-19 vaccination ticket
      • Fill in Prevaccination Screening Questionnaire (Japanese form) before the vaccination. If you have difficulty reading Japanese, please refer to the translations in multiple languages on the website of Ministry of Health, Labour and Welfare.
        *Use one sheet for one vaccination
      • Please do not remove the COVID-19 vaccination ticket. Do not stick it on the Prevaccination Screening Questionnaire.
      • Visit your vaccination venue during your vaccination time slot (20 minutes). Do not visit before your reservation time slot to avoid congestion at a vaccination venue.
      • COVID-19 vaccines are injected in your shoulder. Please wear clothes that make it easy to access your shoulders such as T-shirt.
      • Measure your body temparature at home. If you have a fever of 37.5 ℃ or over, you cannot take the vaccine.

If you cancel the COVID-19 vaccination for some reason

      • You made a rservation by phone: Contact the call center at 092-580-1892.
      • You made a reservation online: Cancel online. If you find it difficult to cancel online, please contact the call center.

What to Bring on the Day of Your Vaccination

1. Prevaccination Screening Questionnaire
*COVID-19 vaccination ticket is attached on the prevaccination screening questionnaire.
*Use one sheet for one vaccination.
2. Document to identify yourself such as residence card, valid driver’s license card, health insurance card, passport, My Number Card (Individual Number Card)
3. Medicine record handbook (If you have)

Vaccination Procedure at Vaccination Venue

Please check the procedure for smooth vaccination.

1. Check Your Body Temperature

We check your body temperature at the entrance and ask you to disinfect. If you have a fever of 37.5℃ or over on your vaccination day, you cannot ge vaccinated.

Submit your vaccination ticket and document to identify yourself such as valid driver’s license card, health insurance card

3. Check Prevaccination Screening Questionnaire

We check whether you fill all items in your prevaccination screening questionnaire.
Please fill in the prevaccination screening questionnaire beforehand to avoid congestion at a vaccination venue.

4. Pre-examination

Pre-examination is conducted by a Doctor.

5. Vaccination

COVID-19 vaccines are injected in your shoulder. Please wear clothes that make it easy to access your shoulders such as T-shirt.

6. Issuance of a Certificate of Vaccination for COVID-19

We will issue a certificate of vaccination for COVID-19

7. Observation

Observation for 15-30 minutes following the instruction by a doctor.
After the vaccination, you keep staying at the vaccination venue for 15-30 minutes to observe your health condition. If you feel something wrong with your condition, please let a nurse near you know.

If you feel something wrong with your condition after leaving the vaccination venue, please call Fukuoka Prefecture Dedicated COVID-19 Vaccine Phone Number and consult.

Possible Side Effect

Side effects such as pain on the arm you get shot, headache, muscle pain, fatigue, fever after the COVID-19 vaccination are reported. There is a rare case, but the life-threatening allergic reaction, anaphylaxis, is reported as well.
